  • It is off-label. Dutasteride mesotherapy is not an approved route of

  • Systemic absorption is not zero. The stated rationale is negligible
    systemic exposure, but the evidence is mixed — one study found absorption sufficient to
    cause changes in sperm count and sperm motility in participants. For men
    trying to conceive, or planning to, that is material information.
  • Absorption into the scalp is immediate and complete by this route,
    compared with roughly a quarter of a topical dose over twelve hours — which is the intended
    benefit, and also why systemic exposure cannot be assumed away.
  • Side effects mirror oral anti-androgens where they occur: reduced
    libido and erectile dysfunction, usually mild and temporary, but potentially taking weeks to
    months to resolve.
  • Study quality is low and nothing is standardised — no agreed needle
    depth, no agreed dose per session, and high variability between patients in how much is
    absorbed systemically.
  • Paradoxical hair loss has been reported following dutasteride
    mesotherapy in a published case series. Uncommon, but documented.

None of that makes it unusable. It makes it a prescription decision
requiring proper counselling — the same conversation you would expect before starting oral
finasteride, which most men are given and most mesotherapy patients are not.

If you are male and planning a family, if you have any history of sexual dysfunction or
mood disturbance, or if you would simply rather not take an anti-androgen, say so. There are

Diagnosis Comes First — Same Rule as Any Hair Treatment

Hair loss is a symptom with many causes, and mesotherapy only helps some of them. Scarring
alopecias such as frontal fibrosing alopecia and lichen planopilaris destroy the follicle
permanently and need prompt anti-inflammatory treatment — injecting cocktails into scarred
scalp achieves nothing while the condition advances. Telogen effluvium usually resolves once
the trigger is addressed. Low ferritin, thyroid dysfunction and vitamin deficiencies need
correcting.

So the same sequence applies here as for

  2. Blood tests where indicated, reviewed before a plan is agreed.
  3. Cocktail decision and full disclosure. Exactly what will be injected,
    including whether any prescription medicine is included, with counselling on it if so.
  4. Numbing where required, then treatment — multiple fine superficial
    injections across the affected scalp. Fifteen to thirty minutes.
  5. Course. Mesotherapy is session-intensive: commonly four to eight
    sessions one to two weeks apart initially, then maintenance monthly or less often. Plan for
    the total, not the first appointment.

Realistic Expectations

  • Reduced shedding is often the first sign, sometimes within four to six
    weeks.
  • Density and thickness changes take three to six months, because hair
    cycles slowly.
  • It supports existing follicles. It cannot create follicles that no
    longer exist, so genuinely bald scalp will not respond.
  • Results are best in early-stage loss. Advanced thinning responds
    poorly.
  • It is ongoing. Stopping generally means gradual return to the previous
    trajectory, because the underlying cause is unchanged.

Recovery and Aftercare

  • Day 0. Scalp tenderness, small bumps at injection points, occasional

  • Do not wash your hair for 24 hours.
