First: Is Your Jaw Width Actually Muscle?
This is the assessment that decides whether the treatment can work at all, and it is
routinely skipped. A wide lower face comes from three possible sources, and toxin only
addresses one:
- Masseter muscle bulk (hypertrophy). Treatable. Clench your teeth and
feel the sides of your jaw — if a firm bulge pushes out under your fingers, you have muscle
to work with. - Buccal fat or general facial fullness. Not treatable with toxin.
Relaxing the muscle will change nothing. - Bone structure — a naturally wide mandibular angle. Not treatable with
toxin either. No injection narrows bone.
Many people have a combination, in which case toxin delivers partial improvement. Being
told this honestly before you pay is the difference between a satisfied patient and someone

Why the Masseter Gets Overdeveloped
- Bruxism — grinding and clenching, usually at night and usually
unnoticed by the person doing it. The muscle enlarges like any muscle worked repeatedly. - Stress and jaw tension, including daytime clenching people are unaware
of. - Chewing habits — gum, tough foods, ice.
- Genetics. Some people simply have larger masseters, which is why this
treatment is especially sought after where a slimmer, more tapered lower face is the
aesthetic preference.
The Functional Side: Bruxism and Jaw Pain
This is the half of the treatment that gets undersold, and for many patients it is the
more valuable half.
Relaxing the masseter reduces the force it can generate. That means less grinding damage,
less clenching, and relief from the symptoms that follow:
- Waking with a tight, aching or tired jaw
- Tension headaches, particularly at the temples and in the morning
- Jaw pain when chewing, and TMJ discomfort or clicking
- Tooth wear, flattening, sensitivity or cracked teeth your dentist has flagged
- Facial and ear-adjacent pain with no dental cause
Functional relief arrives faster than cosmetic slimming — often within two weeks. It works
well alongside a dental night guard rather than instead of one: the guard protects the teeth,

Two Different Timelines — This Matters
Unlike treating lines, where everything is settled at two weeks, this treatment has two
separate clocks:
- Functional relief: 1–2 weeks. Clenching force drops, jaw tension and
grinding-related headaches ease. - Visible slimming: 4–12 weeks. Slow, because you are waiting for a
large muscle to shrink from reduced use. There is nothing to see at two weeks, and that is
normal — not a failed treatment.
Duration: 4 to 6 months, longer than facial toxin. And it improves with
repetition — because sustained reduced use causes genuine muscle atrophy, most patients need
two or three sessions to reach their full slimming result, then maintenance at longer
intervals. Do not judge this treatment on one session.
Your Appointment
- Consultation and examination. You will be asked to clench repeatedly
while the muscle is palpated on both sides, to gauge bulk, strength and asymmetry — most
people are noticeably stronger on one side. Grinding symptoms, dental history, medications
and any TMJ diagnosis are reviewed. - Muscle mapping. The safe injection zone is marked. Placement here is
critical, and it is discussed below. - Plan and consent. Units per side, expected timeline for both effects,
risks and price — agreed before anything begins. - Injection. Deep into the muscle, typically three to five points per
side. The masseter is thick, so doses are considerably higher than facial areas — commonly
around 20 to 30 units per side, adjusted to bulk and often unequal between sides. - Review. Functional effect at two weeks; slimming assessed at six to
twelve weeks, when the next session is planned.
Risks Specific to Masseter Treatment
These differ from facial toxin risks, and placement is the reason.
- Chewing fatigue. Common and expected for the first one to two weeks —
tough steak, hard bread and chewy foods feel harder work. It settles. - Asymmetric or altered smile. The most significant risk, from injecting
too high or too superficially, where the toxin can affect the muscles that lift the mouth
corner. Temporary, but it is why the safe zone is mapped and injections placed deep. - Paradoxical bulging. An unusual outcome where part of the muscle bulges
on clenching because the deeper portion is treated unevenly. Correctable. - Asymmetry, since the two sides are rarely equal to begin with —
addressed at review. - Over-hollowing with long-term use. Years of aggressive dosing can
reduce the muscle too far, giving a gaunt or sunken look at the sides of the face. This is
a reason for conservative dosing and sensible intervals rather than a reason to avoid
treatment. - Also possible: bruising, tenderness at injection points, mild headache
for a day or two.
Who Should Not Have It
Not suitable if you are pregnant or breastfeeding; have a neuromuscular
disorder such as myasthenia gravis, Lambert-Eaton syndrome or motor neurone disease; have a
known allergy to botulinum toxin; or have an active infection at the injection site.
Tell the doctor before booking if you take aminoglycoside antibiotics,
muscle relaxants or blood thinners; have a diagnosed TMJ disorder — treatment may still help
but needs proper assessment; have significant existing chewing difficulty or dental
problems; have a bleeding disorder; have had jaw surgery or facial implants; or have had
previous masseter treatment, including when and how much.
Aftercare
- Stay upright for four hours.
- Do not massage or rub the jaw area for 24 hours.
- No gym, sauna, steam room or hot yoga for 24 hours.
- Go easy on very tough or chewy food for the first two weeks while chewing strength
adjusts. - Avoid gum and ice chewing — it is part of what built the muscle.
- Keep using your night guard if you have one.
- No facials, peels or microneedling for two weeks.
