Two very different situations bring people to this treatment. Most are elective — someone
unhappy with a result, living with migration, or wanting a clean reset after years of
accumulation. A small number are emergencies, where filler has compromised blood supply and

Hyaluronidase is a prescription medicine administered following a face-to-face
consultation. It is not suitable for everyone.

Critical: It Only Dissolves Hyaluronic Acid

This is the single most important thing to establish before you book, and the reason your
first question at consultation will be what you were injected with.

Hyaluronidase dissolves: hyaluronic acid fillers — the great majority of
what is used today — and HA-based skin boosters and bio-remodelling products.

It does nothing at all to:

  • Poly-L-lactic acid (PLLA) and calcium hydroxylapatite
    (CaHA)
    collagen stimulators
  • PMMA, polyacrylamide gel and other permanent fillers
  • Silicone, including industrial silicone injected illegally
  • Polycaprolactone (PCL) products
  • Threads of any material
  • Fat grafting

If you do not know what you were injected with — and a great many patients genuinely do not
— say so. It is important clinical information, not something to be embarrassed about.
Non-HA products need entirely different management: steroid injection, other medical
measures, or occasionally minor surgery.

Reasons People Choose to Dissolve

  • Lip migration. The commonest reason by far — product above the
    vermillion border creating a shelf or duck-lip appearance. Adding more makes it worse;
    dissolving and rebuilding is the fix.
  • Overfilling. Volume accumulated over years of top-ups that individually
    seemed reasonable.
  • Lumps and nodules that have not settled.
  • Under-eye puffiness from tear trough filler in unsuitable tissue —
    often years old, since under-eye filler lasts far longer than most people are told.
  • Tyndall effect — the blue-grey tinge from superficial placement under
    thin skin.
  • Asymmetry that adjustment has not resolved.
  • Heaviness around the mouth or nasolabial region from direct filling of
    folds.
  • A clean reset. Some patients simply want to start again and see their
    own face before deciding anything.

All of these are reasonable. None require justification.

Emergency Dissolving: Know the Signs

If filler blocks or compresses a blood vessel, hyaluronidase is the treatment that can
save the tissue — and time matters enormously. Prompt administration, ideally
within the first hours and certainly within 48 hours, is associated with substantially better
outcomes. Delay risks necrosis, permanent scarring, and in rare cases vision loss.

Seek help immediately — the same hour — if after any filler treatment you
develop:

  • Severe or escalating pain, disproportionate to normal tenderness
  • Skin turning white or blanched, then mottled, purple, grey or dusky
  • A spreading dull ache beyond the injected area
  • Any change in vision — blurring, dark spots, loss of field, or double vision
  • Coldness of the skin over the area

treatment uses far higher doses than elective dissolving, flooded through the affected area

The Risk You Must Understand: Allergy

Hyaluronidase carries a real, if uncommon, risk of allergic reaction — including, rarely,
anaphylaxis. This is the principal risk of the treatment and the reason it belongs in a
medical facility with emergency drugs and trained staff, never in a salon.

Higher-risk patients include anyone with a history of bee or wasp sting
allergy, since hyaluronidase occurs naturally in bee venom, and anyone with multiple drug
allergies or previous anaphylaxis. Tell us about any of these.

Patch testing. For elective dissolving, a small test dose can be given in
the forearm and observed for around 20 to 30 minutes before proceeding. Where the situation is
elective, there is no reason not to take that precaution and we will discuss it with you.

In a genuine vascular emergency the calculus changes — delaying treatment to patch test can
cost tissue, so treatment proceeds with resuscitation facilities immediately available. That
is a clinical judgement made in the moment.

The Part Nobody Warns You About

Hyaluronidase does not distinguish between injected filler and the hyaluronic acid your own
skin naturally contains. It temporarily breaks down both.

The consequence: for the first few weeks the treated area can look more deflated,
more hollow and frankly older
than it did before you had any filler at all. Patients
are frequently distressed by this, having expected to simply return to baseline.

Your native hyaluronic acid regenerates over roughly two to four weeks, and the area
recovers. But you need to know it is coming, and you need to plan for it — do not book
dissolving in the fortnight before a wedding, a holiday, or anything photographed.

Your Appointment

  1. History. What product, when, where, how much, how many sessions, and
    who placed it. Bring records, receipts or product names if you have them — even a
    photograph of the box helps.
  2. Examination. Where product actually sits, which is often not where you
    think. Migration is frequently more extensive than patients realise.
  3. Honest advice, including whether dissolving is the right answer. It is
    not always — see below.
  4. Allergy discussion and patch test where appropriate.
  5. Treatment. Hyaluronidase injected into the target areas, dose and
    distribution according to product volume and location. Fifteen to thirty minutes.
  6. Review at two to four weeks. More than one session is common,
    particularly with large volumes or older, well-integrated product.

When Dissolving Is Not the Answer

An honest clinic will sometimes advise against it:

  • When the product is not HA. It will not work, and you will have taken
    an allergy risk for nothing.
  • When your filler is nearly finished anyway. If it is close to resorbing,
    waiting a few months may be preferable.
  • When the real problem is elsewhere. Heaviness at the nasolabial fold may
    reflect insufficient cheek support rather than too much fold filler — dissolving the fold
    could make things look worse.
  • When lumps are settling. Recent lumps often resolve without
    intervention.
  • When expectations are the issue. If the filler is well placed and
    proportionate, dissolving will not deliver what you are hoping for, and that conversation is
    more useful than the injection.

Recovery

  • Hours 0–48. Swelling, sometimes significant, plus redness and
    tenderness. Lips in particular can swell markedly.
  • Days 2–7. Swelling settles and the loss of volume becomes apparent.
  • Weeks 1–3. The flat or hollow phase, as native HA rebuilds.
  • Weeks 2–4. Recovery toward your genuine baseline.
  • Week 4 onward. The earliest sensible point to consider any new
    filler — and often better to wait longer.

Aftercare: stay upright for four hours; expect swelling and use cool
compresses; no makeup for 12 hours; no gym, sauna or steam for 48 hours; no alcohol for 24
hours; no massage of the area; and no new filler for at least four weeks. Report any
escalating swelling, difficulty breathing, rash or widespread itching immediately — these
suggest allergic reaction.

Risks

Common and expected: swelling, redness, tenderness and bruising; temporary
over-flattening of the area; and the loss of native hyaluronic acid described above.

Less common: incomplete dissolving requiring further sessions; asymmetry
where product dissolves unevenly; over-dissolving, leaving the area flatter than intended
until native HA recovers; and infection.

Uncommon but serious: allergic reaction, including anaphylaxis. This is the
reason for a medical setting, a patch test where elective, and emergency provision on
site.

Who Should Not Have It

Not suitable if you have a known allergy to hyaluronidase; have an active
infection or inflammation at the site; are pregnant or breastfeeding, unless there is a
genuine clinical need such as a vascular emergency; or have a documented previous reaction to
the enzyme.

Discuss carefully before booking if you have a history of bee or wasp sting
allergy; have had anaphylaxis to anything; have multiple drug allergies; take blood thinners;
have an autoimmune condition; do not know what product you were injected with; or had
permanent or unidentified filler placed at any point.
