Start With the Formulation, Not the Machine
Clinics market this treatment on the device. The device is the easy part — every clinic in
Dubai has a needling pen. What differs between a good outcome and a bad one is the cocktail,
and cocktails are not standardised.
Facial mesotherapy formulations typically draw from four ingredient groups:
- Hydrators. Non-cross-linked hyaluronic acid, glycerol. The most
reliable component and the one doing most of the visible work. - Vitamins and cofactors. B-complex, vitamin C, vitamin E, coenzymes.
Antioxidant and supportive. - Amino acids and nucleotides. Glycine, proline, lysine and others —
collagen building blocks — plus nucleic acid fractions. - Minerals and peptides. Zinc, copper, magnesium, and in premium
formulations signalling peptides.
Concentrations, ratios and inclusion vary widely between manufacturers, and some products
are compounded rather than supplied pre-mixed. That variability is why this treatment ranges
from genuinely useful to essentially decorative depending on where you have it.
Ask for the product name and manufacturer. Ask to see the box. Ask whether
anything is mixed on site. A clinic that answers vaguely is telling you the cocktail
is not the part they thought about.
Why the Channel Route Changes the Pharmacology
Applied to intact skin, most of a mesotherapy cocktail goes nowhere useful. The stratum
corneum blocks molecules above roughly 500 daltons, and hyaluronic acid, peptides and nucleic
acid fractions are all substantially larger than that. A serum containing them is a good
moisturiser and little more.
Micro-channels bypass that barrier — which is the entire point of combining the two. But it
cuts both ways, and this is the part rarely discussed:
The channels do not discriminate. Everything in the formulation reaches the
living dermis, including components that were only ever tested for topical use, and including
anything contaminating the product. You are not applying a cocktail to your face; you are
delivering it into tissue.
That single fact should raise your standards for product provenance considerably above what
you would accept in a serum — and it is the reason the next section exists.
The Risk That Belongs Specifically to This Combination
Mesotherapy has a documented infection problem. A systematic review of
mesotherapy-associated non-tuberculous mycobacterial infections found these
are a recognised complication, and noted that mesotherapy is frequently administered by
unlicensed providers. Patients typically developed multiple localised lesions
without systemic illness, and most required antibiotic therapy — many needing repeated courses
to clear. Facial mesotherapy has separately been linked to granulomatous
inflammation where technique is poor or products unregulated.
Now consider the delivery route. Injected mesotherapy places a cocktail in
a limited number of discrete deposits. The needling version distributes it through thousands
of open channels across the whole treated area. If the product is contaminated or contains
something the dermis should not meet, the channel route gives it far more access.
This is not an argument against the treatment. It is an argument for being genuinely
fussy about four things:
- A named, UAE-registered product — supplied sealed, opened in front of
you, nothing compounded on site - A licensed medical facility with a medically qualified operator
- A single-use sterile needling cartridge, unwrapped in front of you
- Sterile technique throughout, not salon hygiene
And afterwards: report any lesion, nodule or area of redness that appears days or
weeks later. Delayed multiple lesions are exactly how the infection pattern above
presents, and these organisms are slow to diagnose and stubborn to treat.
Shallow Infusion or Deep Work? Two Different Appointments
The same equipment and the same cocktail can produce two quite different treatments,
depending on depth — and clinics often sell them under one name and one price.
Shallow Infusion Protocol
Needle depths around 0.25 to 0.5mm. The goal is delivery, not injury — enough channel to
get the cocktail in, minimal trauma. Redness for a few hours, essentially no flaking, back to
normal the same day. This is the “meso-glow” version, appropriate for hydration, dullness and
fine texture, and repeatable at short intervals.
Deep Protocol
Depths from around 1mm upward, set by area and indication. Now the needling itself is doing
significant work — collagen induction and, over acne scarring, disruption of fibrous
tethering. The cocktail becomes a supportive layer rather than the main event. Expect two to
five days of redness and light flaking.
Why it matters: if your concern is dullness and dehydration, the shallow
protocol delivers most of the benefit with none of the downtime, and paying for a deep session
is paying for recovery you did not need. If your concern is acne scarring, a shallow infusion
session will do very little and you need the deep protocol — or better,
microneedling
with PRP, which has meta-analysis support for scars, or
RF microneedling
if laxity is involved.
Ask which protocol you are being booked for. It should be a clinical decision based on your
concern, not a default.
Timing: Why the Cocktail Goes On During, Not After
Micro-channels begin closing within minutes of being created. A cocktail applied only at
the end of the session, as a soothing finish, is largely sitting on skin that has already
begun to seal — which puts you back to the 500-dalton problem.
Done properly, the cocktail is applied continuously through the passes and again
immediately afterwards, while patency is highest. If a provider needles the whole face first
and then applies product, they have converted an infusion treatment into a needling treatment
with an expensive serum on top.
What It Realistically Achieves

- Brighter, less dull skin and better light reflection

- Better absorption of your home routine for some weeks afterwards

What It Will Not Achieve
- Volume or contour — that is
filler - Lifting or tightening — laxity needs RF microneedling or surgical
assessment - Removing dynamic wrinkles — that is
botulinum
toxin - Clearing melasma or pigmentation, whatever brightening ingredients the
cocktail lists - Treating active acne — needling over inflamed lesions spreads bacteria,
so acne is controlled first

acne status, Fitzpatrick type and melasma history. Medical history, allergies and

Recovery
- Shallow protocol: redness for a few hours, mild tightness, back to

then dryness and light flaking around days three to five. - Both: no makeup for 12 to 24 hours; gentle cleansing only; no retinol,
acids or exfoliants for one week; no gym, sauna, steam or swimming for 48 hours; and daily
high-factor sunscreen — freshly channelled skin in Dubai’s UV pigments readily.
Risks
Common and expected: redness, mild swelling, dryness, flaking with deeper
protocols, and tenderness.
Less common: allergic reaction to a cocktail component — a more realistic
concern here than with single-ingredient treatments, both because formulations contain many
substances and because the channel route delivers all of them into tissue; post-inflammatory
hyperpigmentation, particularly on Fitzpatrick IV to VI with deeper protocols or inadequate sun
protection; cold sore reactivation, so declare a history; and prolonged redness.
Uncommon but important: granulomatous inflammation, and non-tuberculous
mycobacterial infection presenting as delayed multiple localised lesions — both associated
with unregulated products or poor technique rather than with the concept.
Who Should Not Have It
Not suitable if you are pregnant or breastfeeding; have active acne, skin
infection or a cold sore in the area; have a known allergy to any cocktail component, which
requires the full ingredient list; have a history of severe allergic reactions or anaphylaxis;
or have a keloid or hypertrophic scarring history.
Discuss carefully before booking if you have multiple drug or food
allergies; are immunosuppressed, which raises infection risk meaningfully; have taken
isotretinoin recently; have melasma; have eczema, psoriasis or rosacea in the area; take blood
thinners; have diabetes or impaired healing; have recent sunburn or a fresh tan; or have had
mesotherapy elsewhere with an unknown product.

Priced per session, and the honest comparison is total course cost — shallow protocols
typically need three to four sessions, deeper protocols the same but spaced further apart,
with maintenance thereafter.
Two things to watch. First, a shallow infusion session and a deep session are not the same
treatment and should not necessarily be the same price — ask which you are getting. Second,
unusually low pricing on this treatment should prompt a question about the cocktail, since
unregulated and compounded formulations circulate, and the channel route is precisely the
wrong delivery method for a product of unknown origin.
