The Anatomy — and Why It Is Treated in Layers

The temple has several distinct tissue layers, and the vessels run in the middle of them.
This is the defining safety fact of the treatment.

Between skin and bone lie the superficial temporal fascia, a loose areolar layer, the deep
temporal fascia and the temporalis muscle. Running through the middle strata are the
superficial temporal artery and its branches, the middle temporal vein and the sentinel vein.
Deeper still are the deep temporal arteries.

Which is why filler is placed in one of two safe planes and never in between:

  • Deep — supraperiosteal, directly on the bone beneath the muscle, at a
    defined injection point measured from the temporal crest and the zygomatic arch. Firmer
    product, structural support.
  • Superficial — subdermal, just beneath the skin above the fascia, using
    softer or diluted product for smoothing.

The middle layers, where the vessels live, are avoided entirely. Depth here is not a matter
of preference — it is the safety technique.

The Risks Specific to Temples

Common and expected: swelling for two to five days, bruising, tenderness,
a feeling of fullness or pressure for the first days.

Specific to this area:

  • Visible or palpable lumps. Temple skin is thin and stretched over a
    concave surface — irregularities show and can be felt more readily than elsewhere.
  • Chewing discomfort. If product is placed within the temporalis muscle,
    the temple can ache when you chew. Correct planes avoid this.
  • Headache or pressure sensation for a few days.

Serious and rare — vascular events. Two distinct mechanisms make temples
notable in the literature:

  • Arterial occlusion. Temporal vessels connect with the circulation
    supplying the eye, so occlusion can cause tissue damage or, very rarely, vision loss.
  • Venous injection. The middle temporal vein is large and low-pressure.
    Product entering it can travel, and cases of embolism have been reported. This is a
    temple-specific risk that does not apply to most other filler sites.

What reduces the risk: injecting only in the two safe planes, at defined
landmarks; small volumes, slowly, at low pressure; aspiration where appropriate; cannula in
the superficial plane; and an injector who knows temporal anatomy in detail rather than
generally.

Hyaluronidase is kept on site with staff trained to administer it

  1. Assessment. Depth and symmetry of hollowing, skin thickness, vein
    visibility, brow position, and overall facial proportion — temples are always assessed

  2. Plan and consent. Plane, product, volume per side, expected result and
    the temple-specific risks — agreed in writing before starting.
  3. Marking. The injection point is measured from bony landmarks. This is
    not eyeballed.
  4. Numbing, then injection. Typically 0.5ml to 1ml per side, adjusted to
    hollowing depth. Deep placement is felt as pressure rather than pain. Fifteen to twenty
    minutes.
  5. Review at two to four weeks, when swelling has resolved and symmetry
    can be judged accurately.

Aftercare

  • Stay upright for four hours; sleep on your back for several nights.
  • Do not press, rub or massage the temples for two weeks.
  • Glasses arms rest directly over the temples — switch to a looser frame
    or minimise wear for the first week where possible, as sustained pressure can displace
    product.
  • Avoid tight headwear, sports headbands, swim caps and headphones that clamp for one
    week.
  • No gym, sauna, steam or hot yoga for 48 hours.
  • No facials, peels or head massage over the area for two weeks.
  • No alcohol for 24 hours.
  • Mild pressure or tenderness on chewing in the first days settles; persistent chewing
    discomfort should be reported.

How Long It Lasts

Twelve to eighteen months, often longer — one of the more durable filler areas, because the
temple barely moves and firm product placed on bone breaks down slowly. Some patients get two
years or more.

The result also tends to look better at three months than at three weeks, as tissue settles
around the product. Judge it at review, not in the first fortnight.

Who Should Not Have It

Not suitable if you are pregnant or breastfeeding; have active infection
or inflammation in the area; have a known allergy to hyaluronic acid or lidocaine; have a
history of severe allergic reactions; or have permanent or unidentified filler already in the
temples.

Discuss carefully before booking if you have very thin temple skin with
prominent visible veins — you may be a poor candidate, or need the superficial plane avoided;
take blood thinners or antiplatelet medication; have an autoimmune or inflammatory condition;
have giant cell arteritis or any temporal artery condition; have had previous temple filler,

To be clear about one thing: temple filler is not a treatment for
